Responsibilities:
Coordinate the development and implementation of care plans for our residents in collaboration with other healthcare professionals
Conduct thorough assessments of resident needs and abilities to develop individualized care plans
Ensure that all necessary documentation is completed accurately and in a timely manner, including MDS assessments and care plans
Work closely with the interdisciplinary team to identify and address resident needs and concerns
Provide education and training to nursing staff on MDS assessment tools and care planning principles
Collaborate with other departments, such as therapy and social work, to ensure comprehensive care
Participate in quality improvement initiatives to enhance the care we provide
