MDS Coordinator RN

About the Role:
As an MDS Coordinator RN, you will play a critical role in ensuring the seamless coordination of patient care by facilitating the Minimum Data Set (MDS) process. Your responsibilities will include:
Collecting and reviewing patient information to complete the MDS assessment
Ensuring accurate and timely completion of the MDS assessment
Maintaining accurate and up-to-date records of patient care
Collaborating with the interdisciplinary team to develop individualized care plans
Providing education and support to patients and families
Participating in quality improvement initiatives to optimize patient outcomes
Requirements:
Current RN licensure in the State of Pennsylvania
1+ years of experience or related experience in long-term care or rehabilitation setting
Ability to work effectively in a fast-paced environment
Excellent communication and interpersonal skills
Strong analytical and problem-solving skills
